Terms and conditions for CAPPA Doula Trainings.
Please complete the registration form and submit payment for your upcoming training. If a training is SOLD OUT, you can still complete the registration form to remain on a waiting list for cancellations or for notification of future training. |
Carefully read the following statements and signify that you agree to the terms listed by marking the box prior to paying for your training.
- I understand that I will need to complete a few homework assignments prior to the workshop.
- I understand that I should not make non-refundable travel reservations until Sonya Duffee has confirmed the workshop at least 2 weeks prior to the course.
- I understand that I may not record the training workshop (audio or video). I may take photographs only with the express permission of the trainer and the participants.
- I understand that I may not photograph any PowerPoint slide, training materials, certificates of completion, or any other intellectual property belonging to CAPPA or Sonya Duffee
- I may not reproduce, publish, or distribute any materials in the training manual
- I may not reproduce publish or distribute any certification materials,other than that for the purpose of fulfilling my certification requirements.
- I may not reproduce, publish, photograph share, email or distribute any materials in the training handouts or manual other than for the purpose of using them form my personal business as a labour doula.
As a participant in this training, I agree to allow Sonya Duffee/CAPPA to do the following:
CAPPA or CAPPA Faculty does not assume liability for adverse reactions to foods consumed or items one may come into contact with while eating at any CAPPA Event, Training or Activity you may attend--whether inside or outside of CAPPA planned events and activities.
Details of Refund Policy:
I understand that training is non-refundable and travel arrangements (if required) should not be made until Sonya Duffee has confirmed the class - usually at least 2 weeks prior to the class start date. We understand emergencies may arise and although we do not issue a refund, your payment can be applied to a future training by the same faculty within one year of initial postponement. Applicant must contact Sonya Duffee to make these arrangements.
If the class is cancelled by Sonya, attendees have the option of receiving a full refund or transferring to another doula training with Sonya Duffee.
Certificate of training completion is given only if the entire training is attended, the Pre-Workshop Study Guide is completed, terms and conditions are received and training fees are paid in full.
- Publish my name, address, phone number, and email address for the trainee class list and referral list.
- Take photographs of the training which may include me, as well as allow the trainer and CAPPA to use these photographs for promotional purposes online and in print.
- Use quotes from my training evaluations (which I may fill out anonymously) for promotional purposes online and in print.
